Evidence: Clinical guidelines ask that assessment and follow-up include information from family and school in more than one setting; the NICE (2018) ADHD guideline, for example, uses observation scales completed by parents and teachers. Keeping a log of sleep, mood, medication and incidents is a widespread practice to help the paediatrician see patterns (Barkley, 2015), but there are no studies showing that this particular diary improves diagnosis or treatment. Achenbach & Rescorla (2001) is a system of standardised questionnaires (ASEBA) that this diary does not replace.

Level of evidence: C (widespread practice, without solid evidence). Basis: Log of symptoms and routines that families bring to the appointment; the NICE guideline (NG87) asks for information from family and school. Limits: No studies show this diary improves diagnosis or treatment. It interprets nothing: the professional does.
This tool is for guidance and educational purposes. It does not replace assessment by a qualified professional.
