Guide for educational transitions. Based on Kohler & Field (2003) and IDEA transition requirements.
Choose transition type
Evidence: Kohler & Field (2003) propose a model of transition-focused education (planning, skills, family involvement and coordination between services); it is a conceptual framework, not an outcomes study. The IDEA (2004) is a US law, so it does not apply in Spain; it requires transition planning from age 16. Wehman (2013) collects good practice for the transition to adult life.

Level of evidence: C (widespread practice, without solid evidence). Basis: Transition planning (Kohler & Field, 2003) and good practice in special education. Limits: Kohler & Field propose a model; they do not measure outcomes. The IDEA law cited is from the US.
